Prurigo y fototerapia, el manejo multidisciplinario del prurigo crónico

Resumen

El prurigo es una enfermedad crónica consistente en la formación de nódulos o pápulas pruriginosas, secundarias al rascado persistente de la zona lesionada, ocasionada por reacciones alérgicas, enfermedades sistémicas y/o metabólicas y que puede afectar en gran medida la calidad de vida de los pacientes que la padecen. De hecho, en la actualidad a nivel mundial, existen escasos estudios epidemiológicos sobre esta enfermedad, considerada poco común. Su prevalencia se estima en 72 por 100.000 habitantes. Sin embargo, en nuestra experiencia, podría ser bastante más frecuente. Es más prevalente en el grupo de edad entre los 50 y 60 años y en personas de raza negra. No se han descrito diferencias significativas entre sexos. Mientras tanto a nivel nacional, se reporta un estudio de corte transversal que incluyó los registros clínicos de pacientes con prurigo atendidos en el Servicio de Fotodermatología del Hospital Universitario Centro Dermatológico Federico Lleras Acosta entre el 2011 y el 2016 (17). Las características de los pacientes colombianos con prurigo son similares a las reportadas en otros países latinoamericanos: inicio de la enfermedad hacia la edad media, predominio en mujeres, con adecuada respuesta al tratamiento tópico y sistémico. A lo largo de la historia se han descrito múltiples tratamientos orales y tópicos, encaminados a controlar el síntoma predominante que es el prurito persistente, así como en mejorar la calidad de vida del paciente tanto en el aspecto físico como emocional. La fototerapia en la actualidad se ha establecido como una opción de tratamiento eficaz en los casos de personas en los que los tratamientos orales o tópicos, antihistamínicos o corticoides están contraindicados ya sea por alergia a estos o por efectos secundarios que puedan exacerbar sus enfermedades de base.
